WELLNESS INCENTIVES

If you are currently covered under any of the MetLife Critical Illness, Accident, and/or Hospital Indemnity Policies, you also receive the “Wellness Benefit” as part of your policy.

​

The Wellness Benefit Amount is $50.00 per calendar year for each person insured under each product.

​

“MetLife will pay Wellness Benefit Amount for one wellness test per calendar year per Insured if the Insured has a wellness test performed while the Insured’s coverage is in force.”

Wellness Tests

Included, but not limited to (please see individual plan certificate for details):

• Annual physical exam

• Biopsies for cancer

• Blood test to determine total cholesterol/triglycerides

• Bone marrow testing

• Breast MRI, ultrasound, sonogram

• Cancer antigen 15-3 and 125 blood test for breast cancer (CA 15-3)/ovarian cancer (CA 125)

• Carcinoembryonic antigen blood test for colon cancer (CEA)

• Carotid doppler

• Chest x-rays

• Clinical testicular exam

• Colonoscopy; Digital rectal exam (DRE)

• Doppler screening for cancer

• Doppler screening for peripheral vascular disease

• Echocardiogram; Electrocardiogram (EKG) • Endoscopy • Fasting blood glucose/plasma test

• Flexible sigmoidoscopy

• Hemoccult stool specimen

• Hemoglobin A1C

• Human papillomavirus (HPV) vaccination

• Lipid panel

• Mammogram

• Oral cancer screening

• Pap smears or thin prep pap test

• Prostate-specific antigen (PSA) test

• Serum cholesterol test to determine LDL or HDL

• Serum protein electrophoresis

• Skin Exam; Skin cancer biopsy; screening

• Stress test on bicycle or treadmill

• Successful completion of smoking cessation program

• Tests for sexually transmitted infections (STIs)

• Thermography

• Two hour post-load plasma glucose test

• Ultrasounds for cancer detection

• Ultrasound screening of the abdominal aorta for abdominal aortic aneurysms

How to file your Wellness Benefit Claim:

  1. Call MetLife at 1-800-438-6388

  2. Provide details including: healthcare provider’s name, address & phone number; screening test, date and address where completed. Multiple claims for spouse or dependent children can be included all in one call if currently one if they are included on your plan.

  1. Complete Accident, Critical Illness, or Hospital Indemnity Claim Form(s)

  2. Mail, Fax or scan & email to MetLife (contact info included on form)
