What is Vision Insurance?

Vision Insurance is health and wellness plan designed to reduce your costs for routine preventive eye care including eye exams and prescription eye wear (eyeglasses and contact lenses).

Plan Rules:

Eligibility: Eligible full-time employees (as described within the Eligibility section of the employee benefits guide), spouse and children (up to age 26)
Coverage through Eyemed
Employee must be actively at work on the effective date
Claims must be submitted within 90 days of the date of service
Additional hearing benefits available
